ACTIVE  SURVEILLANCE  REPORT  OF  OMICRON  POSITIVE  CASE (Manipur)

RAJKUMAR  GITKUMAR  SINGH,  48  YRS/  MALE TERA  YENGKHOM LEIRAK,  IMPHAL  WEST 

The  District  Surveillance  Team,  Imphal  West  comprising  of  Dr.  M.  Vivekson,  DSO  (I/W);  Dr.  H. Wanglensana,  District  Contact  Tracing  i/c  (I/W),  Dr.  L.  Inaocha,  DNO  Testing  &  Tracing  (I/W)  and Dr.  N.  Haraba,  DNO  Admission  (I/W)  have  conducted  active  surveillance  at  the  residence  of Rajkumar  Gitkumar  Singh  (Omicron  Positive  Case)  at  Tera  Yengkhom  Leirak,  Imphal  West  on 27/12/2021. 

The  following  activities  are  carried  out:-

 The team  interacted  with  the  local  club  members,  RK  Romabati  w/o  RK.  Gitkumar  Singh and  neighbouring  villagers. Identified  new  containment  area. Provided  awareness  to  the  ASHA,  CHO  of  HWC  Khamnam  Bazar,  Local  Club  members and  neighbouring  villagers. Physical  Contact  tracing  is  carried  out. 

The  details  of  Contact  Tracing  are:-

   Identified  16  family  members. ACTION  –   All 16 family  members  are  tested  with  RTPCR  on  22/12/2021  and  result  of the  test  came  out  on  24/12/2021;  3  out  of  16  family  members  are  found  to be  positive.  All  negative  family  members  are  instructed  for  14  days  home quarantine.

   Went  for  repairing  cycle  of  his  son  at  a  Cycle  Workshop  at  Sagolband  Salam  Leikai  on 21/12/2021. ACTION  - 3  workers  of  the  cycle  workshop  are  identified  for  testing  and  instructed  for home  quarantine  for  14  days. 

  Went to Pan Dukan of Koijam  Indu  Devi  at  Tera  Yengkhom  Leirak  for  purchase  of  packed drinking  water  bottle. ACTION  -   Koijam  Indu  Devi,  Pan  Dukan  owner  is  identified  for  testing  and  instructed for  home  quarantine  for  14  days.

   Went to meet Mr. Ningthoujam

 Subashchandra at  his  residence  at  Tera  Yengkhom  Leirak on  21/12/2021. ACTION  -   Mr.  Ningthoujam  Subashchandra  is  identified  for  testing  and  instructed  for home  quarantine  for  14  days.

  Some  Tiles  Labours  from  Mantripukhri,  Imphal  East  have  worked  at  his  residence  till 21/12/2021. ACTION  - Cross  notification  is  sent  to  DSO,  Imphal  East  for  their  contact  tracing  and testing.

RAJKUMARI  RIYA,  14  YRS/  FEMALE D/O  RAJKUMAR GITKUMAR  SINGH  (OMICRON  POSITIVE  CASE) TERA  YENGKHOM LEIRAK,  IMPHAL  WEST 

The  District  Surveillance  Team,  Imphal  West  comprising  of  Dr.  M.  Vivekson,  DSO  (I/W);  Dr.  H. Wanglensana,  District  Contact  Tracing  i/c  (I/W),  Dr.  L.  Inaocha,  DNO  Testing  &  Tracing  (I/W)  and Dr.  N.  Haraba,  DNO  Admission  (I/W)  have  conducted  active  surveillance  at  the  residence  of Rajkumari  Riya  at  Tera  Yengkhom  Leirak,  Imphal  West  on  27/12/2021. 

 The  following  activities  are  carried  out:-

   The team interacted  with  the  local  club  members,  RK  Romabati  -  mother  of  RK.  Riya  and neighbouring  villagers. 

  Identified  new  containment  area.

   Provided  awareness  to  the  ASHA,  CHO  of  HWC  Khamnam  Bazar,  Local  Club  members and  neighbouring  villagers. 

  Physical  Contact  tracing  is  carried  out.

 The  details  of  Contact  Tracing  are:- 

  Identified  16  family  members.    Attended  Physics  Tuition  at  NK  Tuition  Centre  at  Kwakeithel  Konjeng  Leikai,  Imphal  West till  21/12/2021  (4:30  am  to  5:30  am  Batch). ACTION  - Identified  54  students  for  testing  and  instructed  for  14  day  home quarantine. 

  Attended  Chemistry  Tuition  at  the  residence  of  Sir  Romeo  at  Kwakeithel  Konjeng  Leikai, Imphal  West  till  21/12/2021  (6:00  am  to  7:00  am  Batch). ACTION  - Identified  28  students  for  testing  and  instructed  for  14  day  home quarantine. 

  Attended  regular  physical  class  at  Tiny  Tots’  Unique  School,  Dewlahland,  Imphal  East  till 21/12/2021  (student  of  Class-IX,  Section-  A) ACTION  - Cross  notification  is  sent  to  DSO,  Imphal  East  for  their  contact  tracing  and testing.

The  District  Surveillance  Team,  Imphal  West  comprising  of  Dr.  M.  Vivekson,  DSO  (I/W);  Dr.  H. Wanglensana,  District  Contact  Tracing  i/c  (I/W),  Dr.  L.  Inaocha,  DNO  Testing  &  Tracing  (I/W)  and Dr.  N.  Haraba,  DNO  Admission  (I/W)  have  conducted  active  surveillance  at  the  residence  of  Mr. Hijam  Ibochouba  at  Tera  Yengkhom  Leirak,  Imphal  West  on  27/12/2021.   

The  following  activities  are  carried  out:-

   The  team  interacted  with  the  local  club  members,  Mr.  Hijam  Chuwang  –  s/o  Hijam Ibochouba  and  neighbouring  villagers.

   Identified  new  containment  area. 

  Provided  awareness  to  the  ASHA,  CHO  of  HWC  Khamnam  Bazar,  Local  Club  members and  neighbouring  villagers. 

  Physical  Contact  tracing  is  carried  out.

    Attended  a  Birthday  Celebration  at  the  residence  of  Mr.  Thounaojam  Shitaljit  at Chingmeirong  (Near  NEXA  Showroom),  Imphal  East  on  17/12/2021. ACTION  -   Cross  notification  is  sent  to  DSO,  Imphal  East  for  their  contact  tracing  and testing.

   Held a meeting  with  local  friends  at  his  residence  on  18/12/2021. ACTION  -   3  persons  are  identified  for  testing  and  instructed  for  14  days  home quarantine.
